Course selection form, step by step
Each student picks their courses; you see the count for each course and the names of the students in it.

Each student picks their courses just once, using their student number; a course that fills up is automatically marked "Full", and in "Reports" you see both the count and the students' names for each course.
🧱 Questions
- 1 "Full name" — "Short text", required This field becomes the name in people reports, so make sure its title contains "Name".
- 2 "Student number" — "Employee ID" (or "Short text"), required This is what "One response per code" is based on; Persian and Latin digits count as the same.
- 3 "Field of study" or "Class" — "Dropdown", required To break the report down by field of study.
- 4 "Selected courses" — "Multiple choice", required One option per course; in "Option details", set a "Capacity" for each course and set the "Minimum selections" and "Maximum selections". If each person takes only one course, use "Single choice".
- 5 (Optional) "My priorities" — "Ranking" Use it if you want priorities too; the "Each person's choices" report shows each priority in its own column.
⚙️ Settings
- 1 "Share → Link settings" → "One response per code": the "Student number" question
- 2 In the same card, set the "Time range" with "From date" and "To date" The form opens and closes on schedule by itself.
- 3 (Optional) The "Settings" tab → "Email notification" → "Receipt for the respondent" Each student gets a confirmation of their selection; the form needs an "Email" field.
📤 Publishing and sending
Click "Publish" and post the link or QR code from the "Share" tab in the class group, on the school's learning portal or on the notice board.
📊 Which report gives you the answer
Count per course: the question card for "Selected courses" in the "Reports" tab (the count and percentage for each option).
Names per course: at the bottom of the same card, "Who chose each option" — each course with its students' names next to it.
List for one field of study: at the top of the page, "Break down by" field of study; for a file, use "Export" in "Responses" to get an Excel file.
🔑 Required plan
Capacity, "One response per code", breakdowns and Excel export are free. The name list for each course is "People reports" (from the "Pro" or "Class" plan up), and if you have more students than the free account's response limit, pick a plan whose monthly response limit covers them; "unlimited forms" doesn't raise the response limit.
